    1. You can change your mind at any time. If you want to withdraw your consent, contact me in writing (email or text is fine). You do not need to give a reason.

    2. I will remove your images within 14 days. If you withdraw consent, I will remove your images from my social media and website within 14 days. Please note that I cannot recall printed materials (such as leaflets or business cards) that have already been distributed.

    3. Your images will not be sold. I will never sell your images to any third party.

    4. Your name will not be used without permission. Your name will not appear alongside any images unless you give separate permission in the section below.

    5. Your images will be stored securely. All images are stored in line with UK GDPR requirements.

    How to Use This Template - Practitioner Notes

    This section is for you, not the client. Remove it before printing if you prefer.

    • Get this signed BEFORE taking any photos. Do not take photos first and ask for consent later.
    • Taking a photo is NOT the same as consent to post it. A client may agree to treatment record photos but refuse social media use. Respect each tick box separately.
    • Keep signed forms for 7 years after the last appointment. Under the Limitation Act 1980, claims can be brought up to 6 years after an event. Keeping records for 7 years gives you a safety margin.
    • If a client withdraws consent, act within 14 days and document it. Save a record of when you received the request, what you removed, and when you completed it.
    • Verbal consent is not enough for GDPR purposes. You need written consent. This form provides that. A verbal "yes" on its own will not protect you if challenged.
    Remember: taking a photo is NOT the same as consent to post it. Get this form signed BEFORE taking any photos.
